On page 2 of the profile update, the immunisation record requires you to enter your child’s immunisation status. Depending on your answer to this question, you may be required to upload an immunisation document to support your selection before you can proceed to the next page.
If you select any of the following, you will be required to upload a document:
- Up to date
- Catch-up Schedule
- Medical exemption
- Not up to date
If you select any of the following, you will not be required to upload a document:
- No immunisation certificate
- Certificate already provided
Often a child has an immunisation certificate, but the parent/guardian completing the profile update does not have it available at the time to upload. If this is your situation, please contact the school to enquire about the best course of action they wish for you to take.
